Clinical Observation of Lower Lung Field Tuberculosis

Abstract
The classic dictum that pulmonary tuberculosis is a disease located predominantly in the apices of the lungs often suggest etiologic agents other than the tubercle bacillus as the cause of isolated lesions in the lower half of the lung fields. Only when tubercle bacilli are demonstrated in the sputum is the correct diagnosis arrived at. The comparatively low incidence of this type of tuberculosis on routine admissions !and outpatient clinics have often made the physician unaware of its existence. Lower lung field tuberculosis was investigated in 6080 patients at Jeon Nam-Bug Province from March 1962 to March 1968. Two Hundred eight-six cases were found, representing 4.7 percent of¢¥ the total cases. A higher incidence of the disease was found in the first and second decades of life, with predominance in the right side and a larger number of females were affected than males (6. 2 percent versus 3.9 percent). The respiratory symptoms, such as cough, sputum, hemoptysis and chest pain were more prevalent in the selected group than in the general group of tuberculosis. The incidence of cavity formation was not so high in the lower lung fied tuberculosis than in the general group of tuberculosis (30. 3 percent versus 25. 2 percent). The cavities in many cases were smaller than 3cm, single and they had no fluid level. The association of the parenchymal lesions with pleural effusion was also rare (5 cases over age 11). No atelectasis was found. Endobronchial disease was present in 138 patients (42. 2 percent over age 11). Family contact history to open pulmonary tuberculosis patient was high percent in young patients than the old. A tendency toward predominant incidence of lower lung field tuberculosis at young age group was one of our observation. Lower lung field tuberculosis is suspected as continuation of primary tuberculosis in our observation.
